Mean cell hemoglobin is a calculated index of the complete blood count: it expresses the average amount of hemoglobin carried by a single red blood cell. It describes how well the marrow is filling each cell with oxygen-carrying pigment, and so speaks for iron supply and for the machinery of hemoglobin synthesis.

Lowered values most often accompany iron deficiency, chronic blood loss, or the thalassemia traits, in which cells leave the marrow underfilled and pale. Raised values are seen most often alongside deficiency of vitamin B12 or folate, in liver disease, and with regular alcohol use, where cells are larger and carry proportionally more pigment. This index is descriptive rather than decisive, and one result outside the range is a reason to repeat the count and discuss it with a doctor, not a conclusion in itself.
Because the value is derived from hemoglobin and the red cell count, anything that distorts either — a difficult draw, a clotted or diluted sample, very high white cell counts, or a drip running in the same arm — shifts it. It is read together with hemoglobin, red blood cells, MCV, MCHC and RDW, since the size, filling and uniformity of the cells only make sense as one picture, and usually with iron studies or B12 and folate ordered separately.
